Across nearly every chronic illness I treat, I keep meeting the same hidden obstacle: a nervous system locked in fight-or-flight. Until the brain comes out of survival mode, even the best protocols struggle to take hold. For centuries, Western medicine has treated the body as a machine: parts to diagnose and repair in isolation from the person living inside. And the same blind spot crept into functional medicine

Integrative and functional medicine corrected much of conventional medicine’s blindness — but inherited the same fragmentation. We simply built a new set of silos. Today there are highly capable specialists for mold and Lyme, for mast cell activation (MCAS), for gut health, for hormones and peptides, for anti-aging and detox. Each silo treats a part. In my experience, none truly addresses the mental-emotional core beyond a referral out. No one treats the brain itself.

When I actually see breakthroughs

Two things have to happen first

1. Treat the brain first. The nervous system has to come out of fight-or-flight and into a calm, regulated state before the body will respond to anything else. Supplements, antimicrobials, and detox protocols all work better when the brain is no longer in lockdown.

2. Address the emotional load. Unresolved trauma, hidden blockages, and psycho-emotional stress have to be met — not bypassed — for real healing to hold. Treat the vitamin, the toxin, or the bug alone, and the breakthrough rarely comes.

The missing piece

So what is the brain actually doing?

To understand how IASIS works, we start with the brain regions and networks that drive how we feel, remember, and respond to threat. The answer begins deep inside — with the limbic system.

The limbic system — the brain’s emotional core and threat detector

Deep in the brain sits the limbic system, the network that processes emotion, memory, and survival. When it senses threat, it drives the fight-or-flight response. An overactive limbic system keeps the body locked in survival mode, flooding it with stress hormones and blocking the calm needed for repair.

The Default Mode Network — the mind turned inward

When you stop focusing on the outside world, a distinct network switches on, linking the medial prefrontal cortex, the posterior cingulate and precuneus, and the angular gyrus. This Default Mode Network (DMN) is not a flaw — it is essential for self-understanding. But when it becomes overactive and rigid, it drives the very patterns that trap chronically ill patients: self-referential thought (“what does this mean about me?”), replaying the past, and projecting worry into the future.

Health is flexibility, not stillness

The DMN is not the only network. A healthy brain flexibly switches between several — and that flexibility is the whole point.

Rest, memory, and self-reflection. Active during downtime.

Goal-directed tasks, problem-solving, and decision-making.

Salience

The switch

Decides what matters and toggles between the other two networks.

Health is moving smoothly between focus, rest, and engagement. Dysregulation is getting stuck.

When the network gets stuck: two patterns we see again and again

The rumination spiral. The mind circles the past, the future, and the worry — looping faster, settling less, until the pattern becomes rigid. Each loop reinforces the one before it, deepening the groove until the brain cannot find its way out.

The trauma loop. The patient can’t move past the trauma — the illness itself, or the overwhelm of the healing journey — until it begins to define who they are. Identity fuses with illness, and the nervous system never returns to baseline.

In our clinic, two very different roads lead to the same stuck place. Sometimes a frightening chronic illness becomes the focus of life: studied constantly, researched obsessively, replayed without resolution, until cortisol rises and the patient slides deeper into fight-or-flight. Other times the driver is biological — toxins or infection create so much neuro-inflammation that the brain over-reacts and locks into a sympathetic-dominant state. The biology and the psychology then reinforce each other. Either way, the brain ends up rigid, inflamed, and locked in threat — and the body cannot heal.

An analogy that helps

Think of the brain as a city

Health is free-flowing traffic. Trauma is gridlock. After trauma, traffic jams into the same few routes — the same thoughts, fears, and stress responses, again and again. Most of the map sits unused while the brain keeps funneling back into the same congestion. A flexible brain, by contrast, moves easily between routes: focus, rest, and engagement open and close as needed, information flows freely, and the system adapts. That free-flow state is exactly what we want to restore.
